Safety, Tom Barkalow

Barkalow reported on three safety issues that were reported during the past week.

  • One incident occurred when a user had difficulty removing an electrical plug from an receptacle, so a pair of needle-nosed-pliers was used to remove the plug. This caused an electrical short but did not trip the breaker. This action could have caused serious injury as this was an energized 208V receptacle. The user institution was asked to follow-up so future like violations could be avoided.
  • Another incident occurred when a small quantity of explosive material was brought to 16-BM in a user's automobile. Even though it was a very small quantity (10 micrograms), Department of Transportation has no minimum amount of explosive material that is acceptable in this type of case.
  • The third incident involved a user grabbing a razor blade by the wrong end with the result being a cut to the hand. This was a case of someone being in a hurry and being careless.
Barkalow said that two out of three of these incidents ended up as ORPS reports. He also emphasized the fact that we have to work with the users to make them aware of safety-related issues and ensure that all user training is up-to-date.
